Tully Hill is accepting applications for an experienced individual to join our team as a SUD Counselor / Case Manager, Outpatient Program. Responsible for managing a caseload of patients seeking outpatient treatment for substance use disorders. Job duties include performing essential counseling functions including screening, intake, orientation, assessment, evaluation, intervention, treatment planning, referrals, crisis intervention, counseling, patient education regarding substance use disorders, individual and group counseling, family counseling, employer collaboration, case management, reporting and record keeping and care coordination. Will work collaboratively with all departments to achieve the best outcomes for patients and families.
Qualified individuals must have the ability to work in a fast-paced environment as part of a multi-disciplinary team. Strong written and verbal communication skills and EMR skills required. Must ensure quality, timeliness and accuracy of documentation to ensure compliance with CARF standards, NYS OASAS regulations, other applicable State and Federal regulations, referral sources and insurance contract requirements. CASAC or NYS Qualified Health Professional qualified in a discipline other than substance use disorder counseling (LMSW, LCSW, LMHC, CTRS, RN, etc) required. Demonstrable knowledge of substance use disorders and OASAS regulations required. Demonstrable knowledge of 12-Step philosophy preferred.
